Japan experiences a syphilis outbreak

Xinhuanet · 新华网

The number of syphilis patients in Japan has stayed high in recent years, while the treatment drug penicillin is in short supply; relief is not expected until the second half of next year at the earliest.

According to Kyodo News on the 9th, the country reported over 1,000 cases in 2013, rising to nearly 8,000 in 2021 and more than 13,000 in 2025.

Experts believe the disorderly growth of social media and dating apps has increased sex with unspecified multiple partners, and is a major cause of the spread of syphilis in Japan.

In Japan, penicillin is the first-choice drug for treating syphilis, and the only recommended drug for blocking transmission from mother to child.

The report says that because of problems with the US firm Pfizer's product and production equipment last year, penicillin is currently scarce on the Japanese market. The company expects shipments to resume in the second half of next year.

Syphilis is caused by Treponema pallidum and can damage the nervous and cardiovascular systems; its routes of transmission include sexual contact. A pregnant woman who is infected may pass it to the foetus.

03 / BEFORE YOU READ ON

Do you think
all of this is true?

There is no scoring here. Both answers lead to the same place.

04 / WHAT ACTUALLY HAPPENED

The number
was falling.

2,950notifications, Japan, Q2 2026

3,748the same quarter, one year earlier

Japan’s own national surveillance puts the second quarter of 2026 at 0.8 times the year before. Syphilis notifications there are going down, and were going down on the day the headline ran.

Nothing in the wire copy was invented. The 13,000 cases are real. The penicillin shortage is real. Pfizer’s equipment failure is real. Every sentence survives a fact-check on its own.

What was added is a word — 暴发, outbreak — and an explanation the wire it cites never gave. The explanation is not a Chinese invention; Japanese public-health pages have named dating apps since 2022. But Japan hedges it, and Japan’s surveillance records sex, age, prefecture and route with no field for apps at all. What arrives here is that hedged guess with the hedges taken off.

Quarterly figures from Japan’s national infectious-disease surveillance. A frame is not a lie. It is a selection made out of true things, and it is much harder to argue with.

05 / TWENTY-SEVEN HOURS

One wire.
Four headlines.

Kyodo filed at 08:31 on 9 September. Xinhuanet ran it at 15:59 the same day. By 10:21 the next morning the story was moving under a different word, and by that afternoon the explanation had moved out of the second paragraph and into the headline. Four tellings, twenty-seven hours, nothing invented at any step — and at each one somebody chose what the sentence was about.

Four headlines, each checked against the published text. Read it sideways.

9 — 10 SEPTEMBER 2026

07 / KEPT AND REPLACED

Carried over and accurate

  • 13,000 cases in 2025
  • Two-thirds of those infected are men
  • Pfizer's recall, and the equipment failure
  • Penicillin is the first-line treatment
  • Mother-to-child transmission — one clause, and one line at the end

Put in its place also not false

  • A drug shortageAn outbreak
  • A machine broke at PfizerDating apps grew disorderly
  • Doctors asked to ration the drug for pregnant womenNot carried
  • Stillbirth, premature birth, congenital syphilisNot carried
  • Notifications at 0.8× the year beforeNever mentioned

Nothing on the left was dropped for being wrong, and nothing on the right was made up. One column is what happened; the other is what the story was made to be about. Only one item here has no source at all — and it is the one that supplies the cause.

08 / AN UNNAMED SOURCE

Who said
the apps
did it?

Experts believe the disorderly growth of social media and dating apps has increased sex with unspecified multiple partners, and is a major cause of the spread of syphilis in Japan.

It is attributed to a report that does not contain it. The paragraph opens according to Kyodo News, 9 September. Every figure in it matches that report. The sentence about apps appears nowhere in it.

Only one of its words is new. 不特定多数 — unspecified numbers of people — is Japan’s own public-health wording, on prefectural health pages and in a Diet question. 无序, disorderly, is not. We looked for it in Japanese public-health and news sources and did not find it; the Japanese pages that carry 無秩序 are translating Chinese coverage back.

The dispatch itself is signed. Xinhua’s copy is datelined and carries a staff journalist’s byline, as wire copy should. The expert inside it does not have a name. We know who wrote the sentence down; we do not know who is supposed to have said it.

The data has no such field. Japan’s surveillance records sex, age, prefecture and route of transmission. There is no column for dating apps. Named Japanese sources say a careful version of this — one of the causes, not clearly understood. The unhedged version has no name on it.

Stuart Hall says a message is encoded with a preferred reading — the meaning it invites you to take — and that the preference is built into the language rather than announced. 专家认为 is that construction in two words. It offers no evidence, only a warrant, and asks to be decoded as science says so. Nobody had to intend it. [16]

On the main reading, every person living with HIV speaks under a research code. They have no names because the study protects them. The expert here has no name either — the same anonymity, doing the opposite work. It shields the stigmatised from exposure, and the author of a stigma from account.

09 / WHERE IT CAME FROM

The sentence
has a history.

It was not invented in China. Japanese public health has named dating apps since 2022 — carefully, every time. Watch what happens to the care.

  1. Kagawa Prefecturehealth office, Japan

Why infections rose so quickly is not clearly understood. That the spread of SNS and matching apps has increased opportunities for sexual contact with unspecified numbers of people is thought to be one of the causes.

Two hedges in two sentences. It does not know why, and apps are one cause among several.

  2. Xinhuanet6 January 2026

The development of social media and dating apps has led to more sex with unspecified numbers of people; this is a major cause of the spread of syphilis in Japan.

Both hedges gone. 考えられます — thought to be — becomes 导致, led to. One cause among several becomes a major cause.

  3. Xinhuanet9 September 2026

The disorderly development of social media and dating apps has led to more sex with unspecified multiple partners, and is a major cause of the spread of syphilis in Japan.

Eight months later, the same outlet, the same sentence — plus one word. 无序: disorderly.

In May 2023 Japan’s own dating-app industry association objected in public that this reporting linked apps to syphilis 因果関係を示すデータや具体的根拠が何ら示されないまま — with no data or concrete grounds for causation shown at all. The objection was to Japanese coverage. [10]

10 / THE COUNT

Two headlines
out of a hundred and two.

Every Chinese-language item three of us could find in four days: 245 of them, 191 distinct headlines, 132 posted by self-media accounts. 102 were published by media organisations. Each mark below is one of those 102; the dark ones called it an outbreak.

  • 53of 102 headlined an outbreak
  • 18named dating apps in the headline
  • 64distinct headlines among the 102

In the body36 of 42mentioned pregnancy, mother-to-child transmission or congenital syphilis

In the headline2 of 102did the same

Kyodo published on 9 September because Japan’s Society for Sexually Transmitted Infections had asked doctors to keep the remaining drug for pregnant women. Chinese coverage almost always kept that in the text. Almost none of it put it where people read.

Searched 9–12 September 2026 on five fixed queries, two of them neutral so that accurate reporting could surface. Headline codes are a string match on the published headline — 暴发 爆发 疫情 激增 失控 蔓延 — so anyone can rerun them. Bodies were read for 42 of 60 items drawn at random: 37 gave dating apps as a cause, 34 credited an unnamed expert, 1 mentioned that notifications were falling, 0 gave any rate or comparison. Body coding is a first pass, not yet double-coded. The 18 that could not be opened were mostly WeChat search results, which return expiring redirect links rather than permanent ones. Weibo is excluded — too few of its items were institutional to belong in this denominator — and Toutiao was not collected, because its search is personalised enough that three people would not have been searching the same thing. [02]

11 / WHAT THIS SHOWS

A frame is not
a lie.

Syphilis has always been sexually transmitted. Saying so is not a stigma; it is a fact about a bacterium. The stigma begins one step later, when a route of transmission is promoted into an explanation — and the explanation offered is a moral one.

In Japan’s version this is a disease that needs a drug: a machine broke, and doctors were asked to keep what was left for pregnant women. In the Chinese version it is a behaviour that needs discipline. Same facts. The fault has moved from a manufacturer to the people who caught it.

That move is the whole of it. The main reading follows the identical operation performed on HIV over twenty-four years: 高危人群, a category of conduct standing in for a virus — and 501 articles out of 643 in which nobody was a person.

One story is not evidence of a pattern; the 643-article analysis on the main reading is what carries that claim. This piece does something narrower. It shows the mechanism did not stop, and that you can watch it work in public, on a Thursday, without anyone lying to you. [10][16]

THE CHAIN / STAGE 01 OF 04

9 Sep, 08:31 · Kyodo News

THE DRUG

Syphilis treatment drug, supply shortage continues — equipment failure at plant, resumption next year

The news is a broken machine. Pfizer recalled the drug in July 2025 and its production equipment failed that November. Japan's Society for Sexually Transmitted Infections has just asked clinicians to keep the remaining stock for pregnant women.

Social media and dating apps are not mentioned anywhere in the report.

9 Sep, 15:59 · Xinhuanet

THE PATIENTS

Japan's syphilis patients increase while medicine runs short

Eight and a half hours later, datelined 新华社北京9月9日电 and signed. The shortage is still in the headline but it is no longer what the sentence is about; the patients have moved to the front of it. Every figure is Kyodo's, and correctly attributed to Kyodo.

暴发 is not in this copy. The dating-app sentence already is — in the body, credited to an unnamed expert.

10 Sep, 10:21 · China News Service, syndicated

THE EPIDEMIC

Japan experiences a syphilis outbreak

The word is in neither earlier version. Pfizer has left the headline, and so has the drug.

Twenty-seven hours after Kyodo filed. Carried by Shenzhen News, Guangming, The Paper, NetEase and MSN, word for word.

10 Sep · Sina Finance · 21st Century Herald

THE CAUSE

Japan experiences a syphilis outbreak — linked to the penicillin shortage and the disorderly growth of social media and dating apps

Nothing here is new. The expert's sentence has been in the body since 15:59 the day before; what changed is where it sits. A cause in paragraph two is a detail. A cause in the headline is what the story is about.

Same day. The claim did not appear — it was promoted.
